In this presentation Annette Hubbard shares on the innovative and patient centered mobile care that is happening at Ninilchik Traditional Council Community Clinic. The mobile clinic provides life saving harm reduction services, pharmacologic treatment for substance use disorder, connection to recovery programs, screening for infectious disease, primary care and many additional health care services. This program shows an impressive model of relational and person centered care that is inline with the unqiue characteristics and challenges that rural communities can face.

Annette is an Alaskan Native in long term recovery who has personal experience with harm reduction treatment.  She currently works as a medication assisted treatment case manager for a tribal health clinic in rural Alaska and has worked there since October 2016.  She provides education regarding medication assisted treatment, harm reduction, overdose prevention, mental health services, and substance user health care.
